Health Form Information To all parents of day camp participants: This notice is to advise you as to the importance of completing the Woodbury Day Camp Health History Form in its entirety.
As the director of the volunteer-led day camp in Woodbury, my first priority is keeping all day camp participants healthy and safe. The best way for me to enable our volunteers to do that is by having a completely filled out and accurate health history form on file for each participant. All information on the form remains confidential except in the event of a medical emergency, when health history forms are given to emergency care providers. Please know that by submitting fully completed and accurate forms, it not only helps our volunteer health managers, but anyone else who may serve our participants and their health needs, as well.

All new adult volunteers must complete a background check via the secure website:
www.intellicorp.net/portals/GSWI . If you've completed a background check previously, you don't need to do it again. Enter password "meffen92" which is case sensitive.
Swingin' to the Stories Daycamp 2010
This year is all about your favorite book characters! Who will you meet? Will they be fictional characters or real-life people? You'll just have to wait and see!
Daycamp is for girls entering grades 1-6 in the fall of 2010 who want to be active outdoors, learn some new skills, and make new friends while still being able to go home each night. Except for Thursday when all campers are invited to sleep over. Girls who are not a current registered Girl Scout are invited to attend and will need to register to become a member at the same time you register for day camp.
Activities: Most days you will cook over a fire, make crafts, sing songs, and swim.
When and Where: June 14-18, 2010 (Monday - Friday) Camp Rolling Ridges in Hudson, Wisconsin
Please remember that no registration is complete unless the registration form, fees, health form and insurance card for EACH person are mailed to the address indicated. Make checks payable to "Woodbury Service Unit Day Camp".
REFUND POLICY: Up to April 30, 2010, 100% of the fee paid will be refunded. From May 1 to May 15, 2010, 50% of the fee paid will be refunded. After May 16, 2010, refunds will be determined on a case-by-case basis.
